Step to Add Security Questions to WordPress Login Page
Here is how you can add security questions to your WordPress login page.
 Go to your WordPress dashboard and choose Plugins from the left panel.
 Choose Add New and search for WP Security Questions in the search bar.
 Click on Plugin from the search result and click on Install Now.
 After installation, click on Activate Now to activate the plugin.
 Once Activated, expand the Settings menu from the dashboard.
 Choose WP Security Questions and choose Settings under it.
 Check the box next to the options to enable Security Questions in the login page, registration page, and forget password page.

- Set security answers on registration screen
- Display security question on forget password screen
Note: It is not necessary to check all the boxes. You are free to choose the ones you need.
 Once the boxes are checked, set up the questions to add to the page.
 Once you set it up, click on Save Settings to store all the changes done.

How to Add Security Questions Manually to the Users
Apart from adding security questions in common, you can also add security questions manually for any particular user by using the same WP Security Question plugin.
 Install and Activate the WP Security Questions plugin on your WordPress.
 Go to Users from the WordPress dashboard.
 Choose Profile and here you can see the My Security Questions section.
 Choose any one question from the drop menu and answer it.
 Click on Update Profile and the user will be asked to answer the security question during every login.
